How It Works

Enter patient information, add multiple respondents, select a form, and send a link to your clients within minutes.

Your client follows a link in their email or text to their secure patient dashboard, where they complete assigned tasks such as a client history or release of information.

You receive notification that tasks have been completed. Log in to your practice dashboard and download the narrative client history in .docx or .pdf format.
Service Option 1: A la carte
If you prefer to pay per form, you can purchase a single use for $15.
Service Option 2: Monthly Subscription
You can use HACCAH with up to 20 clients at a rate of $50/month. If your client base is larger than 20 clients per month, contact us for your monthly rate.
Service Option 3: Bespoke Forms and Reports
If our existing forms don't fully meet your needs, we can work with you to create unique forms and reports to address the specific needs of your client population. For these services, we charge $150/hour. We will work with you to estimate the time that may be required for your specific needs.
